Melbourne City Mission’s service profile includes Youth, Adult and Family; Disability; Employment and Education; Homelessness; and Palliative Care.
The Community Integration and Accommodation Options (CIAO) program has two areas of focus: Lead Tenant and Targeted Care Packages (TCP). The CIAO program serves young people under the age of 18 and aims to assist them to develop the skills necessary for healthy and sustainable, independent living within their community of choice. CIAO facilitates a beneficial and meaningful transition from Out-of-Home Care (OOHC) toward self-sufficiency. The CIAO program is committed to assisting young people to develop skills, build personal capacity, and to access the services necessary for sustainable and healthy independent living. Using a holistic approach, the CIAO program targets interventions to the many facets of adolescent development and wellbeing.
The Case Worker will provide support and services to young people in receipt of Targeted Care Package funding who are transitioning from the OOHC system to adulthood.
